Air India has fired its pilot following a positive psychoactive substance test linked to a severe altitude upset that injured passengers on flight AI-2379.

The airline confirmed on Friday that the Pilot-in-Command psychoactive substance test resulted in a positive finding, leading to immediate dismissal. The incident occurred on August 4, when the Airbus A320 experienced catastrophic hydraulic system failures while cruising at 36,000 feet. The Aircraft Accident Investigation Bureau (AAIB) released a preliminary report detailing the sequence of technical failures that preceded the physical distress experienced by those on board. While the autopilot disengaged and stall warnings sounded, the pilot remained on duty despite the subsequent medical findings. The AAIB described the confirmation of the substance use as a serious concern, urging the Directorate General of Civil Aviation to take priority action to prevent future abuse of such substances.
The technical breakdown of the aircraft was swift and severe, occurring at 04:02:43 UTC. According to the AAIB’s preliminary report, the flight-control system first detected a loss of pressure in the green hydraulic system. Just four seconds later, losses were detected in both the blue and yellow hydraulic systems. This rapid cascade of failures caused the autopilot to disengage at 04:02:48 UTC, immediately followed by a two-second stall warning. The co-pilot, who was piloting the aircraft at the time, initially responded to the alarms before the Pilot-in-Command took over the controls.
During this critical window, the aircraft experienced a significant altitude upset. The plane gained 372 feet before falling 292 feet from its assigned flight level. The hydraulic systems eventually recovered, with the blue system stabilizing around 04:02:52 UTC, followed by the yellow and green systems. This restoration allowed the flight-control surfaces to function normally again, returning the aircraft to a stable operating condition. However, the brief period of instability was enough to cause substantial physical damage and injury, as the seat-belt signs had been off during the upset.
The lack of active safety warnings led to severe consequences for those inside the cabin. Upon landing in Delhi, medical personnel attended to 20 passengers and four cabin crew members. The injuries ranged from minor to serious, with 20 individuals discharged after treatment for minor ailments, while four people with serious injuries were hospitalized. The flight crew assessed the situation and made the decision to continue to Delhi rather than divert to a nearest en-route airport, requesting medical priority from Delhi Air Traffic Control. The aircraft landed safely at IGI Airport at 05:30 UTC.
The physical toll of the incident was visible in the cabin's condition. The incident caused significant damage inside the cabin, including cracked and deformed ceiling panels and dislocated passenger-service-unit panels. The preliminary report emphasized that both pilots underwent post-flight medical checks after the aircraft landed in Delhi. While their breath analyser tests were satisfactory, the Pilot-in-Command's sample was found non-negative in a psychoactive substance detection test. A subsequent confirmatory blood test was also found non-negative, whereas the co-pilot tested negative.
This finding has triggered a mandatory response from the aviation authority. The AAIB stated that the confirmation of the use of a psychoactive substance is a serious concern and recommended that the Directorate General of Civil Aviation take appropriate action on priority. The goal is to prevent the abuse of such substances in the cockpit. The detailed investigation remains in progress, with investigators focusing on both the mechanical failures of the hydraulic systems and the human factors involved.
